HHSC Corporate Office – Atherton Building
Instructions: This self-administered self-assessment tool is to be completed by HHSC corporate employees every day on which the employee plans to be working on the Leahi Campus.
While on the Leahi Campus, Corporate Employees shall: wear masks at all times unless in an office separated from others; stay in the Atherton Building and walkways to and from the parking lot or drop off area, corporate VTC, elevators, bathrooms, and hallways to and from these areas; employees shall maintain 6 foot separation at all reasonable times. Employees may not use elevators, hallways, vending machines, etc. outside of Atherton.
